E4M9: Fear is the secret map of Thy Flesh Consumed in Doom and can be accessed from E4M2: Perfect Hatred. It was designed by Tim Willits and uses the music track "Hiding the Secrets".

At the start, open the door in front of you. Collect the Shotgun in here, and open the other door in here too. Kill the Zombiemen in front of you, and from the door, hit the switches to the right and the left. When you do, turn back around, go through the doors again. When you get out of the second door, go straight, and when you get into a hallway, go right. When you get to a door (not the one to the left wall), and right. See those pillars to the right that have openings in between them? Go to the last section of pillars from where the door we stopped at. Get in the opening. Save your game. See that thin platform? Jump carefully on it. You can run if you need to, but if you miss the platform, load your saved game. When you do make it, go left, and walk slowly to the Yellow skull key. Then get into the water and take the teleporter.
On hard skill levels, you may telefrag a Baron when you get here. In this little "cage", flip the switch on the lump. Then, go right, then run to the door. Then, in here, take the staircase on the right, and kill the Shotgun Guy at the top, and then open the door. Kill the Imps and Shotgun Guys in here. From the door, go straight and take the lift down. When you get to the bottom, open the door in front of you. Go through, and then left. Follow this hallway to a door on the left wall. Open it. Go through. Follow this hallway to a Yellow Door, killing any Demons, Imps, Zombiemen, or Cacodemons in your way. Open the Yellow Door, and get to the exit switch (do not flip it). Facing the exit switch, turn around, and check the walls. When part of the wall opens, get the Backpack inside. Now go back and hit the exit switch.

- Once in the large circular room in the south, go through the door to the west and continue right up the stairs. After passing through the next door, turn around and open the differently textured wall just left of it. Inside is a berserk pack.
- Open the wall directly across from the exit switch to get a backpack.

In multiplayer games it is possible to get stuck in the cage located at the centre of the southern egg-shaped area. This is because the switch to lower the cage walls is blocked by a short green pillar decoration that only appears in multiplayer. The only way for the stuck player to get out of the cage is to die and respawn elsewhere or if another player lowers the cage walls from outside of the cage.
